Registering a new Provider account

Fill out W-9

Locum Life's W-9 forms are powered by Adobe Sign. To find the link to fill out your W-9, go to your account page by clicking on the user icon next to your username in the main menu. Scroll down near the bottom of the page, and you will see a section for your W-9 with a link, Submit W-9. Click this link and fill out your W-9 form on Adobe Sign.

Fill out ACH deposit form

Locum Life's ACH forms are powered by Adobe Sign. To find the link to fill our your ACH direct deposit authorization form, go to your account page by clicking on the user icon next to your username in the main menu. Scroll down near the bottom of the page, and you will see a section for your ACH form with a link, Submit ACH Deposit Form. Click this link and fill out your ACH direct deposit authorization on Adobe Sign.

Fill out timesheet

Locum Life's timesheets are powered by Adobe Sign. To find the link to fill out your timesheet, go to your account page by clicking on the user icon next to your username in the main menu. Scroll down near the bottom of the page, and you will see a section for your facilities. Locum Life requires you to fill out a new timesheet for each facility, so if you're assigned to more than one facility, you will see multiple links entitled Submit Timesheet - each form appears next to its corresponding facility. Click each of these links and fill out your timesheet for each facility separately on Adobe Sign.
